Step #1

Choose one of two ways to apply:

  1. Print and fill out the application at home. Then mail or drop them off at a local office.
  2. Go to the nearest local office and they will have application materials ready for you.

To find your local office, you can use this search tool. Search for Developmental Disabilities Services close to your City, County, or Zip Code.

Step #2

While in person at your local office you will need to fill out an I-9 form in person to complete your application.

Step #3

Once you submit your application you’ll get an email to fill out your background check. Follow the online steps right away to keep your application moving.
