Step #1

Choose one of three ways to apply

  1. Fill out and sign an electronic document online.
  2. Print and fill out the application materials at home. Then mail or drop them off at a local office.
  3. Go to the nearest Aging and People with Disabilities office with the Department of Human Services and they will have application materials ready for you.

To find your local office you can use this search tool. Search for “physical disability services” or “older adult services” close to your City, County or Zip Code.

Step #2

When you complete your application you will need to go into a local office to complete an I-9 form in person.

Step #3

Once you submit your application you’ll get an email to fill out your background check.  Follow the online steps right away to keep your application moving.
